Blunted response to hypercapnia: synonymous with depressed respiratory drive?

نویسنده

چکیده

The pathophysiology of failure to wean from mechanical ventilation consists of depressed central respiratory drive, impaired respiratory mechanics, respiratory muscle weakness, increased load, impaired gas exchange, and compromised cardiovascular performance. Among patients with weaning failure, depressed central respiratory drive occurs in only a small portion (approximately 10%),1 whereas most patients demonstrate augmented drive with the development of progressive ventilatory failure.2

Influence of prenatal nicotine exposure on development of the ventilatory response to hypoxia and hypercapnia in neonatal rats.

In a recent study (Huang YH et al. Respir Physiol Neurobiol 143: 1-8, 2004), we showed that prenatal nicotine exposure (PNE) increased the frequency of spontaneous apneic events on the first 2 days of life in unanesthetized neonatal rats. Here we test the hypothesis that PNE blunts chemoreceptor reflexes. Ventilatory responses to three levels each of hypoxia (inspired O(2) fraction: 16, 12, and...

Hypercapnia test as a predictor of success in spontaneous breathing trials and extubation.

BACKGROUND The ventilatory capacity of the respiratory neuromuscular system can be studied with the hypercapnia test. OBJECTIVE To determine whether decreased response to the hypercapnia test is associated with failure to pass a spontaneous breathing trial (SBT) or extubation failure. METHODS We studied 103 intubated patients ready for SBT. We used a hypercapnia test in which we approximate...

Arousal and ventilatory responses during sleep in children with obstructive sleep apnea.

Abnormal central regulation of upper airway muscles may contribute to the pathophysiology of the childhood obstructive sleep apnea syndrome (OSAS). We hypothesized that this was secondary to global abnormalities of ventilatory control during sleep. We therefore compared the response to chemical stimuli during sleep between prepubertal children with OSAS and controls. Patients with OSAS aroused ...

Effect of Regular Yoga Practice on Respiratory Regulation and Exercise Performance

Yoga alters spontaneous respiratory regulation and reduces hypoxic and hypercapnic ventilatory responses. Since a lower ventilatory response is associated with an improved endurance capacity during whole-body exercise, we tested whether yogic subjects (YOGA) show an increased endurance capacity compared to matched non-yogic individuals (CON) with similar physical activity levels. Resting ventil...

Role of Respiratory Drive in Hyperoxia-Induced Hypercapnia in Ready-to-Wean Subjects With COPD.

BACKGROUND Hyperoxia-induced hypercapnia in subjects with COPD is mainly explained by alterations in the ventilation/perfusion ratio. However, it is unclear why respiratory drive does not prevent CO2 retention. Some authors have highlighted the importance of respiratory drive in CO2 increases during hyperoxia. The aim of the study was to examine the effects of hyperoxia on respiratory drive in ...
